What do I need to know about my insurance to get coverage for a vision test?

Your insurance company requires us to ask prospective patients for a state issued identification (i.e. driver’s license) and a copy of the card your vision insurance provider mailed to your last known address.  Having this information available when you call to set up an appointment helps us to give you information about your copays, if any, as well as the total coverage available to you.  It also allows us to get an authorization from your insurance provider so that we can bill them for the services they are contracted to pay instead of charging you when you come in for your eye exam.
